Participation & Choice - Be involved in setting goals and making decisions in therapy. Ask questions, ask for more clarification, pause or stop any exercise or questions at any time, refuse any treatment or exercise suggestions.
Speak Up - Share feedback or concerns with me without fear. You can also contact the College of Registered Psychotherapists of Ontario, if necessary.
Continuity & Referrals - Be informed about changes in your care or receive a referral elsewhere, if needed.
Access to Your Information - With a few exceptions, see your record and get clear answers about your care. If we are unable to give you access, we will provide a written explanation within 30 days.
